Biplane Intertrochanteric Osteotomy of the Femur in Young Age Group averages 20 years.



Abstract
Hip problems such as osteoarthritis, INFH (Idiopathic Necrosis of Femoral Head) in young age group averages 20 years have many difficulties in treatment.
Many techniques have been developed such as osteotomy, arthrodesis & cementless total hip arthroplasty for the treatment of hip problems in younger adults. But, a concern that prosthetic hip replacement has failed more frequently in young adult leaves these patient with no choice but to palliate pain and compromise their activities until their remaining life expectancy.
We have performed 7 cases of biplane intertrochanteric osteotomies for the hip problems of young age group. The average follow-up period was 8.7 months and the range of age at operation was 12-30years.
Of 7 intertrochanteric osteotomies, 3 cases were varus-flexion type, 2 cases were varus-extension type & the other 2 cases were valgus-extension type. The average of the Harris hip score was 73 preoperatively and it was improved up to 89 postoperatively. In most of cases, patients were pain free or with pain that was only minimal or at least considerably less than prior to surgery. In spite of short follow-up period and small number of cases, we experienced satisfactory results especially in pain relief and the results showed promising expectancy in the further follow-up period. In view of these findings we believe that in younger adults, biplane intertrochanteric osteotomy should be considered initially for the treatment of osteoarthritis & osteonecrosis of the hip.
